Blaze Advisors was formed in 2016 to offer a suite of talent and tools to help healthcare partners optimize their pay-for-performance programs. We took a pragmatic, innovative approach to identifying realistic and impactful solutions for our customers. Services often included assembling and analyzing disparate population health data, implementing and automating care management programs, and designing and deploying communal disease/risk screening tools.
In 2017, Blaze Advisors was asked to build and manage integrated care networks as a core project, now known as ONEcare, that coordinated the historically siloed medical and behavioral health communities. Using straightforward communication protocols and tools, ONEcare networks sought early identification of disease, timely access to care, and robust treatment activation and patient engagement.
By 2022, we realized we had outgrown our name, so we changed it to Alera Health. Alera – which is Latin for “eagle” – suggests agility, grace, power, and strength. We believe it reflects how our multifunctional advisory and management team works with hospitals, emergency rooms, primary care, behavioral health, and post-acute care providers across the nation. We are proud to help our partners design and accelerate care innovation that helps save lives, one population at a time.
We are a group of cross-trained experts with extensive, professional experience creating high-performance networks for behavioral health, post acute care, and community-based organizations. Our team is clinically and operationally integrated across the healthcare continuum, including an array of traditional medical providers and specialty sector providers. This real-life experience based on successes and lessons learned in the management of at-risk payment programs and the pursuit of effective results give us the insights we need to help our partners improve patient care by creating uncommonly good collaborations.
