Network Assemblage & Governance

Alera Health organizes local providers into a self-governed, high-performance ONEcare network. To support contract performance, Alera manages network and national quality improvement committees, empowered with data and analytics. In addition, Alera provides practice transformation support and helps to engineer a ONEcare neighborhood through partnerships.

Alera Health selectively invites capable and proven provider organizations to apply to a ONEcare NetworkOnce network adequacy is achieved, the ONEcare Network is legally formed as a CIN and Alera Health solicits value-based contracts with MCOs.  The participating providers elect a Board of Managers to oversee the performance improvement activities of the network and ultimately hold each other accountable for individual and collective performance. 

Clinical Quality Improvement (CQI) Committee

The workhorse of every ONEcare Network, the CQI Committee identifies priorities, evaluates and implements best practice, provides technical support, and monitors network performance. In addition, the CQI committee will promote shared resourcing and identify High Value Collaborators with which the network seeks to partner. Each CQI committee sends delegates to a National Quality Improvement Committee (NQIC) to expand and translate learnings nationwide.

Data-Driven Insights & Action

Using the reporting and tools provided by Alera Health, each ONEcare network is able to focus resources on high-opportunity and impactable members to derive the greatest population impact.  With these reports, providers are able to quickly identify “chase lists” for outreach, open preventative care gaps, high/rising cost claimants, complex polypharmacy, and even real-time reports on members presenting in the ED or an inpatient setting.

Building the ONEcare Neighborhood

For payers, MCOs, hospitals, and even primary care CINs, a ONEcare network is a one to many connection to high-quality, integrated and standardized behavioral healthcareONEcare Networks provide timely and coordinated access to behavioral healthcare in exchange to reciprocal access to primary and specialty care. 

Hospitals who allow ONEcare providers to participate in discharge coordination find they can save up to 20-30% in uncompensated charity care related to behavioral health admissions. Finally, ONEcare and Alera Health partner with contracting MCOs; aligning benefits and network incentives to build resilient and effective integrated systems of care that benefit their entire membership.

Contracting/Actuarial Support

Alera Health maintains standardized fee for service and value-based contracts with most national and many regional MCOs. These agreements define the attributed population, expected outcomes, baseline and target performance. In addition, Alera Health maintains a broad clinical and knowledge database for actuarial comparisons and projections.

Predicting future performance requires careful understanding of your attributed population and the macro and local factors likely to influence performance and spend in the future. Once understood, these factors can be calculated into contract structure and negotiations to create fair and achievable contracts.

Care Communications and Coordination Technologies

Care coordination requires a lot of communication and ONEcare networks have access to a suite of technologies to automate communication and tracking:  

ONEinsight is Alera Health’s enterprise data warehouse and population health analytics platform. ONEinsight ingests and normalizes data from multiple sources, applies AI and machine learning, and delivers real-time dashboards, alerts, and predictive analytics.

The result: Networks gain clarity to drive performance, close care gaps, and improve outcomes.

With a few clicks, the care coordinator and team can send and receive messages, alerts, and referrals, minimizing confusion and delays caused by “phone tag.”
