Alera Health was founded in 2017 to address a growing gap in population health care: Members with co-occurring behavioral health conditions are 10x less likely to receive preventive medical and behavioral health care, translating to 3x more ED/hospital visits, a cost of care 4-6x higher, and die 25 years sooner, on average.
Because of the diverse behavioral, medical, and social needs of this population, Alera Health began organizing multi-disciplinary “ONEcare” networks willing to outreach, engage, and coordinate care in exchange for performance incentives rewarding improved preventative care and reduced health care expenditures.
ONEcare Networks harnesses local behavioral health and integrated care providers willing to assertively outreach and educate members who would otherwise be forgotten.
Alera Health provides network management, actuarial and analytic support, and QI/practice transformation services. Collectively and individually, ONEcare Networks partner with payers/MCOs, health systems, and primary care networks to build systems of care for measurable and durable impacts.
